Home Health Occupational Therapist OT Part Time

Position Overview

Seeking an Occupational Therapist (OT) to assess, teach, and deliver therapy interventions based on physician-directed home health plans. Collaborate with a multidisciplinary team, providing updates to optimize patient outcomes.

Key Responsibilities

• Perform initial and ongoing assessments of mobility, coordination, and function.
• Administer, evaluate, and interpret tests to set functional goals and outcomes.
• Develop written evaluations and plans of care outlining therapy needs, goals, and progress.
• Deliver occupational therapy services following physician orders.
• Maintain and update the plan of care, document performance, modify goals, and plan discharges.
• Coordinate discharge planning, including patient and caregiver education on home exercises.
• Collaborate with healthcare team members and document according to policies.
• Supervise relevant personnel (e.g., OTA, HHA) as applicable.
• Complete OASIS assessments where permitted by regulation.

Qualifications & Skills

• Degree in Occupational Therapy from an accredited program.
• Current, unrestricted state license as an Occupational Therapist.
• Valid CPR certification.
• Experience in home health therapy and OASIS documentation preferred.
