About the Role

Seeking an Occupational Therapist (OT) to assess, teach, and deliver therapy interventions based on physician-directed home health plans. Collaboration with interdisciplinary team members is essential to optimize patient outcomes.

Responsibilities

  • Conduct initial and ongoing assessments of mobility, coordination, strength, and function.
  • Administer, evaluate, and interpret tests to set functional goals and outcomes.
  • Provide written evaluations and care plans outlining therapy needs, goals, and progress.
  • Deliver occupational therapy services according to physician orders.
  • Maintain documentation, modify treatment plans as needed, and determine discharge plans.
  • Coordinate services with healthcare team, patients, and caregivers, including education on home exercise plans.
  • Supervise other personnel such as OT Assistants and Home Health Aides, if applicable.
  • Complete OASIS assessments where permitted by regulation.

Qualifications

  • Degree in Occupational Therapy from an accredited program.
  • Current, unrestricted state license as an OT.
  • Valid CPR certification and driver's license.

  • Experience in home health therapy and OASIS documentation.
