About the Role
Seeking an Occupational Therapist (OT) to assess, teach, and deliver therapy interventions based on physician-directed home health plans. Collaboration with interdisciplinary team members is essential to optimize patient outcomes.
Responsibilities
- Conduct initial and ongoing assessments of mobility, coordination, strength, and function.
- Administer, evaluate, and interpret tests to set functional goals and outcomes.
- Provide written evaluations and care plans outlining therapy needs, goals, and progress.
- Deliver occupational therapy services according to physician orders.
- Maintain documentation, modify treatment plans as needed, and determine discharge plans.
- Coordinate services with healthcare team, patients, and caregivers, including education on home exercise plans.
- Supervise other personnel such as OT Assistants and Home Health Aides, if applicable.
- Complete OASIS assessments where permitted by regulation.
Qualifications
- Degree in Occupational Therapy from an accredited program.
- Current, unrestricted state license as an OT.
- Valid CPR certification and driver's license.
