Home Health Occupational Therapist Part Time PRN

Position Overview

We are seeking an Occupational Therapist (OT) to assess, teach, and deliver therapy interventions based on physician-directed home health plans. The role involves collaborating with an interdisciplinary team, providing updates to optimize patient well-being, and delivering services aligned with physician orders.

Responsibilities

• Perform initial and ongoing assessments of mobility, coordination, strength, and functional status.
• Administer, evaluate, and interpret tests to set functional goals and outcomes.
• Develop and document plans of care outlining occupational therapy needs, goals, progress, and expected results.
• Deliver therapy services as prescribed, maintaining timely documentation and modifying treatment plans as needed.
• Manage discharge planning, including education on home exercise programs for patients and caregivers.
• Coordinate with healthcare team members, physicians, and families, serving as a resource for clinical staff.
• Complete OASIS assessments where permitted by regulation and document effectively to prevent claim denials.
• Supervise other personnel, such as OTAs and HHAs, as applicable.

Qualifications

• Degree in Occupational Therapy from an accredited program.
• Current, unrestricted state license as an Occupational Therapist.
• Valid CPR certification and driver's license.
• Preferred skills include strong organization, communication, attention to detail, time management, and experience in home health therapy and OASIS documentation.
