Home Health Physical Therapist PRN Part Time

Role Overview

A Physical Therapist (PT) provides home health care services, enabling patients to recover from illness or injury in their own homes.

Responsibilities

• Conducting assessments of mobility, strength, and function
• Developing and implementing personalized treatment plans
• Documenting progress
• Collaborating with healthcare teams, patients, and caretakers
• Ensuring compliance with physician orders
• Maintaining accurate documentation
• Supervising staff as needed
• Participating in discharge planning

Qualifications & Skills

• Degree in Physical Therapy from an accredited program
• Current, unrestricted state license
• Valid CPR certification
• PREFERRED: strong organization, communication, attention to detail, time management, relationship-building, and experience with home health therapy and OASIS documentation

Key Duties

• Performing initial and ongoing patient assessments
• Developing and modifying plans of care
• Delivering therapy services per physician orders
• Documenting effectively with minimal supervision
• Coordinating with healthcare team and educating patients/caretakers
• Supervising staff when applicable
