Position Overview

Key Responsibilities

• Perform assessments of mobility, strength, and function.
• Administer, evaluate, and interpret tests to set functional goals.
• Develop and update plans of care, documenting progress and outcomes.
• Deliver therapy services according to physician orders.
• Collaborate with healthcare team members, document effectively, and supervise personnel if applicable.
• Complete assessments like OASIS where permitted.

Qualifications & Skills

• Degree in Physical Therapy from an accredited program.
• Current, unrestricted state license.
• Proficiency in assessments, documentation, and policy adherence.

Preferred Attributes

• Strong organization, communication, and attention to detail.
• Effective time management and relationship-building skills.
• Experience in home health therapy and OASIS documentation is a plus.
