About the Role
We are seeking an Occupational Therapist (OT) to assess, teach, and deliver therapy interventions based on physician-directed home health plans. The role involves collaborating with interdisciplinary teams, performing evaluations of mobility, strength, and function, and developing personalized care plans.
Responsibilities
- Perform assessments and determine functional goals
- Develop and implement treatment plans
- Document clinical progress effectively
- Coordinate with healthcare team and educate patients/caregivers
- Supervise relevant staff
- Modify treatment as needed and plan for discharge
- Complete assessments such as OASIS
Requirements
- Degree in Occupational Therapy from an accredited program
- Current unrestricted state license
- CPR certification
- Valid driver’s license and automobile insurance
Preferred experience includes home health therapy and OASIS documentation skills.
