Position Overview

We are seeking an Occupational Therapist (OT) to assess, teach, and deliver therapy interventions based on physician-directed home health plans. The role involves collaborating with interdisciplinary teams, performing assessments of mobility, strength, and function, and developing individualized treatment plans.

Responsibilities

• Conduct initial and ongoing assessments to evaluate patient capabilities
• Develop and implement treatment plans with measurable goals
• Document clinical progress and modify interventions as needed
• Cook up with healthcare team and educate patients/caregivers
• Maintain compliance with documentation policies and supervise staff if applicable

Qualifications And Skills

• Degree in Occupational Therapy from an accredited program
• Current, unrestricted state license as an OT
• Valid CPR, driver’s license, and proof of auto insurance
• PREFERRED: Home health experience and OASIS documentation skills
