Position Overview

Key Responsibilities

• Perform assessments of mobility, coordination, strength, and function.
• Administer and interpret tests to set functional goals and outcomes.
• Develop and document plans of care, goals, progress, and discharge plans.
• Deliver therapy services following physician orders.
• Maintain accurate documentation to ensure compliance and support billing.
• Cookordination with healthcare team, educate patients and caregivers on home exercises.
• Supervise applicable personnel and complete OASIS assessments where permitted.

Qualifications

• Degree in Occupational Therapy from an accredited program.
• Current, unrestricted state license.
• Valid CPR certification.

• Time management and relationship-building abilities.
• Experience in home health therapy and OASIS documentation.
