Home Health Occupational Therapist

Responsibilities

• Assess and evaluate patients' functional status, activities of daily living, and occupational therapy needs in accordance with medical conditions and plan of care
• Provide services within the scope of practice, using evidence-based techniques, and collaborate with healthcare team members
• Monitor and document patient responses to treatment and report changes to supervisors and physicians
• Conduct patient assessments using diagnostic and prognostic functional ability tests

Qualifications & Skills

• Current Occupational Therapy licensure in the practicing state
• CPR certification and valid driver's license with access to reliable transportation
• Ability to work effectively in diverse home environments regardless of client background
• Preferred: Strong multitasking, independence, communication, and organizational skills
