About the role

Join our caring team as a Home Health Occupational Therapist, providing personalized rehabilitation care in patients' homes to enhance mobility, independence, and quality of life.

Responsibilities

• Collaborating with physicians and clinical staff to develop, implement, and revise therapy plans focused on daily living skills, strength, mobility, and adaptive equipment needs.
• Participating in care coordination, discharge planning, and acting as a resource for healthcare team members.
• Training patients' families and staff in managing therapy-related activities such as dressing, feeding, and grooming.
• Submitting evaluations, treatment plans, progress reports, and discharge summaries.
• Providing clear instructions to Occupational Therapy Assistants and Home Health Aides, and overseeing their performance and documentation.

Qualifications

• Unrestricted Occupational Therapy license in the relevant state.
• Valid driver's license, auto insurance, reliable transportation, and current CPR certification.
• Preferred experience: Home health care.

Pay

Compensation ranges from $54.00 to $76.00 per visit/unit, with an annual base pay of $85,400 to $117,500.
