Responsibilities
- Assess patients’ social, emotional, and environmental needs and identify appropriate community resources, in-home support services, and other available assistance.
- Collaborate with physicians, case managers, and interdisciplinary team members to incorporate social and emotional needs into the patient’s Plan of Care (POC).
- Provide counseling, education, and support to patients and families/caregivers regarding social needs, community resources, advance directives, and available services.
- Connect patients and families with appropriate community resources and coordinate services to support patient well-being and independence.
- Monitor and communicate changes in patients’ social and support needs to the care team and document relevant findings in the patient health record.
- Participate in discharge planning and care coordination to support a safe and effective transition from home health services.
- Maintain accurate, timely, and confidential electronic clinical documentation in accordance with Lorian policies, HIPAA, and applicable regulations, while participating in continuing education and in-service programs.
Requirements
- Graduate of an accredited school approved by the Council on Social Work Education.
- Master’s degree from a school accredited or approved by the Council on Social Work Education.
- Valid driver’s license and insured personal vehicle (per Lorian requirements).
- Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.

Pay
This position is based on a pay per visit compensation model. Pay per visit ranges from $90 to $110, commensurate with experience and depending on type of visit.
