Home Health LPN

Responsibilities

• Provide direct patient care using a variety of skills and coordinate services.
• Complete documentation according to policies, procedures, and regulations.
• Follow and implement the plan of care assigned by the Registered Nurse.
• Coordinate homecare services with healthcare team members, patients, families, and community agencies.
• Accept and transcribe physician’s orders accurately.
• Forward clinical notes and progress reports to the Registered Nurse.
• Participate in patient care conferences.

Qualifications & Skills

• Completion of nursing education from an approved school.
• Current LPN licensure in the state of practice.
• At least one year of clinical nursing experience, preferably in medical/surgical settings.
• Ability to independently organize work and deliver effective home healthcare services.
• Strong communication skills, both oral and written.
• CPR certification.
