About the role

The LPN visiting nurse provides nursing services utilizing a comprehensive base of nursing theory and nursing process, and communicates/documents observations and assessments. The role includes assisting RN case managers by keeping them informed of changes in patient status, progress toward goals, and any other pertinent information. Responsible for the outcome of applicable QA/OBQI studies and reviews all processes yearly to ensure they are current and within or exceeding guidelines. Active participant in quality improvement programs, such as improving HH-CAHPS scores and Home Health Compare scores.

Responsibilities

• Adhere to nursing conduct standards as defined by the state.
• Communicate effectively and in accordance with agency requirements.
• Report significant changes in client status to the physician, case manager, and other team members in a timely manner.
• Maintain client records showing systematic assessment, planning, intervention, and evaluation.
• Document legibly and according to agency documentation guidelines and standards.
• Demonstrate competency with technical nursing skills according to personal and legal scope of practice:
  • Assessment skills as applied to the client, family/support system, and environment.
  • Teaching skills according to the client’s needs and within their environment, value system, and physical/emotional status.
  • Medication management and teaching.
• Practice nursing with respect for individual, cultural, and spiritual differences.
• Initiate actions to improve the quality of patient care and take appropriate actions independently in home care situations.
• Initiate referrals as needed to other health team members when delegated by the case manager.
• Assist with QA/OBQI functions and be responsible for the outcome of applicable studies.
• Evaluate all applicable processes yearly with appropriate staff to ensure they align with current guidelines and accepted national practices.
• Promote personal safety and a safe environment for clients and coworkers.
• Demonstrate knowledge of safety/infection control practices by complying with established policies and procedures.
• Demonstrate safe practice in the use of equipment.
• Assess the safety of the environment and take initiative to help prevent accidents and promote safety.
• Willing to work weekends and holidays when scheduled.

Requirements

• Ability to move independently or with reasonable accommodation within the facility and community.
• Physically and mentally able to perform all job requirements.
• Pass background and DMV checks.
• Associate's degree from an accredited school of nursing.
• 0-2 years of experience; geriatrics experience helpful.
• Current LPN licensure in the State of Montana.
• Current CPR certification.
