Home Health LPN

Responsibilities

• Provide direct patient care using a variety of skills and coordinate services effectively.
• Complete documentation in accordance with policies and the patient's plan of care.
• Coordinate homecare services with the Registered Nurse, Patient Care Coordinator, and other personnel to ensure optimal patient outcomes.
• Accept and transcribe physician’s orders, and forward clinical notes to the Registered Nurse.
• Participate in patient care conferences and collaborate with healthcare team members.

Qualifications & Skills

• Graduation from an approved nursing program.
• Current LPN licensure in the state of practice.
• At least one year of clinical nursing experience, preferably in medical/surgical settings.
• Ability to independently organize work and deliver effective healthcare services to homebound patients.
• Strong oral and written communication skills.
• CPR certification.
