Home Health LPN
Position Overview
The Home Health LPN provides skilled nursing care to patients in their homes across multiple counties. This role focuses on delivering high-quality, patient-centered home health services, supporting patients with ongoing medical needs, and ensuring safe, coordinated care while working independently in the field.
Schedule & Work Details
Weekly Hour Guarantee: 40 hours
Schedule: Monday through Friday, 8:00 AM to 4:30 PM
On Call and Weekend Requirement: None
Coverage Area: Limestone County
Mileage Reimbursement: Yes, reimbursed between patient visits from first patient to last patient
Pay Rate: $30-34 per hour
Key Responsibilities
- Provide skilled nursing care to patients in their homes under physician orders.
- Perform routine clinical tasks including medication reconciliation, wound care, vital signs, and patient assessments.
- Accurately document care provided in the electronic medical record (EMR).
- Educate patients and caregivers on disease management, medications, and care plans.
- Communicate effectively with RNs, physicians, therapists, and the interdisciplinary care team.
- Maintain compliance with state regulations, agency policies, and infection control standards.
Required Qualifications
- Active LPN license
- Current CPR certification
- Ability to work independently in a home health environment
- Homecare Homebase is a plus
Pay, Benefits, & Schedule
Pay Rate: $30-34 per hour
