Responsibilities
- Provide nursing care to patients in their homes; including treatments, diagnostic procedures, and patient education based on the plan of care and physician’s orders
- Administer medications and treatments as prescribed by the physician
- Monitor patients' vital signs and report any changes to the physician
- Fulfill duties as outlined in the patients' care plans and suggest any changes needed to the supervisor and physician
- Observe signs and symptoms, reactions to treatment, drug response/interactions, and changes in the patient’s condition; with appropriate follow-up in care and reporting to supervisor or physician as needed
- Helps coordinate care, as needed with patient/responsible party, physician, other disciplines, and or internal and external resources
- Communicate with patients' families and caregivers
- Maintain accurate and up-to-date patient records
- Collaborate with other healthcare professionals to ensure the best possible patient care
- Participates in multi-disciplinary team meetings
- Participates in learning and job-enhancement activities as directed by the supervisor
- Participate in On-Call rotation
Requirements
- Current LPN license in the state of North Carolina
- Minimum of 1 year of experience in home health or a related field
- Excellent communication and interpersonal skills
- Excellent time management and organizational skills
- Great technology skills (use iPad/Laptop to record visit notes)
- Strong commitment to patient care
- Able to work independently and as part of a team
- Valid driver's license and reliable transportation
