At Elara Caring, we’re looking for a Licensed Practical Nurse who wants to bring skilled, personal care to patients in the Palmer and Monson area. You’ll work alongside an RN Case Manager and a supportive clinical team while getting to know the people you care for.
Responsibilities
- Providing nursing care in patients’ homes according to their plan of care
- Monitoring patients and sharing changes with the RN Case Manager
- Teaching patients and caregivers how to manage care at home
- Documenting visits, interventions, and medication updates
- Working with the care team to help patients avoid unnecessary hospital visits
Requirements
- Active Massachusetts LPN license
- Graduation from an accredited nursing program
- At least one year of clinical nursing experience
- Valid driver’s license, reliable transportation, and current auto insurance
- Ability to travel throughout the Palmer and Monson territory
- Ability to sit, stand, bend, lift, and move intermittently, including lifting 50–100 lbs.

Pay
$30–$38/hour, based on experience
Schedule
Full-time, Monday–Friday, 8 a.m.–5 p.m
