About the role

We are seeking an experienced LPN to provide direct patient care in a home health setting, coordinating services with healthcare team members to ensure optimal patient outcomes.

Responsibilities

• Completing documentation
• Following care plans developed by Registered Nurses
• Accepting and transcribing physician orders
• Participating in patient care conferences
• Independent organization of work
• Thorough documentation
• Effective communication skills

Qualifications

• Successful completion of nursing education from an approved school
• Current LPN licensure in the state
• At least one year of clinical nursing experience, preferably in medical/surgical settings
• Ability to provide effective healthcare services to homebound patients
• CPR certification
