HOME HEALTH LPN

About the role

Provides direct patient care, including using a variety of skills and coordinating necessary services for homebound patients recovering from illness, trauma, or surgery. Completes appropriate documentation as required by policy, procedure, and regulation.

Responsibilities

• Follows the plan of care appropriate to the patient’s diagnosis and condition as developed and assigned by the Registered Nurse.
• Coordinates homecare services with the Registered Nurse, Patient Care Coordinator, and other agency personnel to optimize patient care, involving the patient, family, community agencies, and physicians.
• Accepts and transcribes physician’s orders as directed.
• Forwards all clinical, progress, and summary notes to the Registered Nurse for coordination.
• Attends and participates in patient care conferences.

Requirements

• Successful completion of nursing education from an approved school of nursing.
• Current LPN licensure in the state where the branch is located.
• One year of clinical nursing experience, preferably in an acute care medical/surgical setting.
• Ability to independently organize work and provide effective healthcare services to homebound patients.
• Ability to ensure appropriate patient care as supervised by the home health nurse (Registered Nurse) and document thoroughly, accurately, and completely on a timely basis.
• Effective communication skills (oral and written).
• CPR certification.
• Background checks and drug-free workplace compliance.
