HOME HEALTH LPN

About the Role

Provides direct patient care, using a variety of skills and coordinating necessary services. Completes appropriate documentation as required by policy, procedure, and regulation. Follows the plan of care tailored to the patient’s diagnosis and condition, as developed and assigned by the Registered Nurse.

Coordinates homecare services with the Registered Nurse, Patient Care Coordinator, and other agency personnel to optimize patient care for the patient, family, community agencies, and physicians. Accepts and transcribes physician’s orders as directed and forwards all clinical, progress, and summary notes to the Registered Nurse for coordination. Attends and participates in patient care conferences.

Responsibilities

• Deliver direct patient care in accordance with the plan of care established by the Registered Nurse.
• Document all care thoroughly, accurately, and in a timely manner.
• Coordinate homecare services to ensure optimal patient outcomes.
• Transcribe and follow physician’s orders as directed.
• Communicate effectively with patients, families, and healthcare team members.

Requirements

• Successful completion of nursing education from an approved school of nursing.
• Current LPN licensure in the state where the branch is located.
• One year of clinical nursing experience, preferably in an acute care medical/surgical setting.
• Ability to independently organize work and provide effective healthcare services to homebound patients.
• CPR certification.
• Background check and drug-free workplace compliance.

Skills

• Effective oral and written communication skills.
• Ability to ensure appropriate patient care under the supervision of a Registered Nurse.
