About the Role
Provides direct patient care to homebound individuals following an illness, trauma, or surgery, utilizing a variety of skills and coordinating necessary services. Supports patient recovery with independence while adhering to the plan of care developed and assigned by the Registered Nurse.
Responsibilities
- Deliver direct patient care in accordance with the plan of care tailored to the patient’s diagnosis and condition.
- Complete accurate and timely documentation as required by policy, procedure, and regulation.
- Coordinate homecare services with the Registered Nurse, Patient Care Coordinator, and other agency personnel to optimize patient care.
- Collaborate with the patient, family, community agencies, and physicians to ensure comprehensive care.
- Accept and transcribe physician’s orders as directed.
- Forward all clinical, progress, and summary notes to the Registered Nurse for coordination.
- Attend and participate in patient care conferences.
Requirements
- Successful completion of nursing education from an approved school of nursing.
- Current LPN licensure in the state where the branch is located.
- One year of clinical nursing experience, preferably in an acute care medical/surgical setting.
- Ability to independently organize work and provide effective healthcare services to homebound patients.
- Ensure appropriate patient care as supervised by the home health nurse (Registered Nurse) and document thoroughly, accurately, and completely on a timely basis.
- Effective oral and written communication skills.
- Current CPR certification.
- Pass a background check and maintain a drug-free workplace.
