Home Health LPN

Responsibilities

• Provide direct patient care in the home, including assessments, wound care, and medication administration.
• Monitor treatment responses and support care plan implementation.
• Coordinate with RNs, physicians, and pharmacists to ensure comprehensive patient care.
• Accurately document patient information in EMR systems.
• Educate patients and families about health conditions and treatments.
• Collaborate with an interdisciplinary team committed to quality and continuous learning.

Qualifications & Skills

• Valid LPN license.
• Strong clinical skills in patient assessment, wound care, and medication management.
• Excellent communication and teamwork abilities.
• Attention to detail for accurate documentation and compliance.
• Ability to work independently and adapt to a dynamic healthcare environment.
