Home Health Licensed Practical Nurse

Role: Licensed Practical Nurse (LPN) in Home Health providing patient care across all age groups. Responsibilities Assist registered nurses with assessing, planning, implementing, and evaluating patient care according to policies and standards.Provide nursing care tailored to patients' developmental stages and diagnoses.Communicate effectively within the multidisciplinary team and maintain patient and family satisfaction.Organize workload efficiently, demonstrate agency values, and promote program effectiveness. Qualifications & Skills Graduate of an accredited practical nursing program with a valid North Carolina license.Current CPR certification and at least one year of practical nursing experience; home health experience preferred.Strong hand/eye coordination, communication skills, and ability to operate care equipment.Physical ability to lift, transfer, and maneuver patients and equipment.Assess patient conditions, develop care plans, and educate patients effectively.Visual and auditory acuity to perform assessments and communicate effectively. Additional Requirements Exposure to potential hazards such as bloodborne pathogens and environmental risks during home visits.Valid driver’s license and operational vehicle required.Ability to work variable hours, including weekends and holidays, serving diverse populations.
