Home Health Licensed Practical Nurse

Responsibilities

• Provide compassionate in-home nursing care to seniors and adults with chronic conditions.
• Administer medications, perform wound care, and educate patients on health management.
• Monitor patient status and document care in electronic medical records (EMR).
• Collaborate with interdisciplinary team to ensure safe recovery, manage illnesses, and promote independence at home.

Qualifications & Skills

• Valid LPN license.
• Excellent communication and patient care skills.
• Ability to work independently and as part of a team.
• Strong attention to detail and documentation accuracy.
