Home Health Licensed Practical Nurse

Responsibilities

• Deliver in-home skilled nursing care to seniors and adults with chronic conditions
• Administer medications, provide wound care, and educate patients
• Monitor patient health status and document in EMR systems
• Collaborate with interdisciplinary teams to support safe recovery and promote independence

Qualifications & Skills

• Licensed Practical Nurse (LPN) license
• Strong clinical skills in medication management and wound care
• Excellent communication and patient education abilities
• Ability to work independently in a home setting and coordinate with team members
• Compassionate approach to patient care and respect for diverse populations
