Home Health Licensed Practical Nurse (Adult)

Role: Licensed Practical Nurse providing one-on-one home care for adult patients. Responsibilities Deliver skilled nursing services including assessments, medication administration, wound care, and monitoring of chronic conditions.Follow individualized care plans to ensure personalized patient care.Accurately document care activities in compliance with clinical and regulatory standards.Communicate patient status changes effectively to the RN case manager and physician. Qualifications & Skills Valid license as a Practical Nurse.Strong clinical skills in patient assessments, medication management, and wound care.Excellent communication and documentation abilities.Ability to work independently and follow care plans diligently.
