Licensed Practical Nurse (Home Health)

Home Health LPN provides compassionate, evidence-based nursing care to adults in their homes. Key Responsibilities Perform patient assessments and deliver nursing care aligned with person-centered care plans.Administer medications and treatments, including wound care.Monitor chronic conditions and respond to clinical changes.Report patient status changes to the RN and physician.Educate patients and families to support optimal health outcomes.Document care accurately in electronic health records.Coordinate with interdisciplinary behavioral health and social service teams.Ensure compliance with clinical, safety, and regulatory standards. Qualifications & Skills Active LPN licensure and strong clinical judgment.Ability to administer medications, perform wound care, and manage chronic conditions.Clear communication and collaboration with RN/physician and care teams.Proficient documentation skills in electronic health records.Patient-centered approach, empathy, and ability to educate and partner with families.
