About the role
We are seeking Licensed Practical Nurses (LPNs) to provide home healthcare services for Pediatric and Adult patients in their own homes.
Responsibilities
- Utilize the nursing process to plan, implement, and evaluate patient care
- Initiate and adjust care plans
- Administer medications and treatments
- Respond to changes in patient conditions
- Provide healthcare education, care coordination, and discharge planning
Qualifications
- Current LPN license in the practice state
- At least one year of LPN experience
- Current PPD or Chest X-Ray
- BLS certification
