Job Summary
Primary function is to facilitate the referral intake process and coordinate care with the interdisciplinary team, the patient, their family, and the referral source.
Job Qualifications
- Education: Graduate of an accredited school of vocational/practical nursing.
- Licensure: Active, state license as a licensed vocational/practical nurse, or a multi-state license issued by a Nurse Licensure Compact (NLC) member state.
- Current driver’s license.
- Experience: One year of experience as a licensed vocational/practical nurse; home health experience preferred.
- Skills: Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills.
Environmental and Working Conditions
- Works in a routine office environment.
- Noise level may be moderately high.
- The ability to work a flexible schedule and extended hours.
- The ability to travel locally with some exposure to inclement weather.
- Possible exposure to toxic materials, toner, etc.
Physical and Mental Effort
- Prolonged sitting and some standing required.
- The occasional need to lift, pull, carry, and push items weighing up to 50 pounds.
- Frequent need to stoop, kneel, and reach while accessing files.
- Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions.
- Requires hand-eye coordination and manual dexterity.
- Requires excellent problem-solving skills.
