Home Health / Hospice RN Case Manager

We are a locally owned and operated hospice and home health agency celebrating our 27th year, known for very high‑quality, patient‑focused care and low employee turnover. We are seeking a qualified Hospice and Home Health RN Case Manager who shares our values and wants a long‑term position with a great company.

Responsibilities

• Provide skilled nursing care including wound care, catheter care, venipuncture, observation and assessment, medication compliance and teaching, hospice assessments, and other skilled care.
• Create patient‑centered goals with patients, family members, and providers.
• Assess, supervise, and direct patient care.
• Develop plans of treatment.
• Ensure chart compliance with state regulations.
• Coordinate patient care with physicians and other providers.

Requirements

• Licensed as a Registered Nurse by the State of Utah.
• Current Utah driver’s license and automobile insurance.
• CPR certification.
• Minimum of 1 year nursing experience (hospice and home health experience strongly preferred).

Qualifications

• Associate degree (required education).
• Registered Nurse (RN) license.
