Home Health/Hospice RN Case Manager

About the role

Superior Home Care and Hospice, a locally owned and operated agency in Salt Lake City, UT celebrating its 27th year, is seeking a qualified Hospice and Home Health RN Case Manager. Responsibilities

• Provide skilled nursing care including wound care, catheter care, venipuncture, observation and assessment, medication compliance and teaching, hospice assessments, and other skilled care.
• Create patient‑centered goals with patients, family members, and providers.
• Assess, supervise, and direct patient care.
• Develop plans of treatment.
• Ensure chart compliance with state regulations.
• Coordinate patient care with physicians and other providers.

Requirements

• Licensed as a Registered Nurse (RN) by the State of Utah.
• Current Utah driver’s license and automobile insurance.
• CPR certification.
• Minimum of 1 year nursing experience (hospice and home health experience preferred but not required).

Qualifications

• Associate degree in nursing (required).
• Passion for high‑quality patient care, strong documentation skills, and enjoyment of caring for patients in a home setting.
