Home Health & Hospice RN - Case Manager

Qualifications

• Associate’s Degree (A.D.), Diploma, Bachelor’s Degree (B.S.), or Master of Science (M.S.) from an approved school of nursing.
• Minimum of two (2) years’ previous home health or hospice experience or in combination with supervisory experience.
• CURRENT State of Ohio Registered Nurse licensure.
• BLS certification or obtained within 30 days of hire.
• Current valid Ohio Driver’s license.
