Home Health & Hospice Liaison
Responsibilities
- Facilitate seamless care transitions by conducting pre-discharge calls and visits to assess patient needs.
- Obtain consent for services and complete admissions when necessary.
- Build and strengthen relationships with key contacts at assigned facilities, educating them on Ethos' services and benefits.
- Provide patient-centered support, offering a "white glove" experience for patients, families, and referral partners.
- Address challenges and resolve concerns to enhance client and facility satisfaction.
- Create and execute strategic sales plans to achieve admission and census goals.
- Participate in rounding with healthcare teams to identify service needs and opportunities.
- Manage discharge follow-up calls and hospice readiness transitions, collaborating closely with clinical teams.
- Develop and implement annual education plans in partnership with marketing, sales, and clinical teams.
- Attend facility events to promote Ethos' services and enhance visibility.
Requirements
- Current clinical license in the states of Minnesota and North Dakota is preferred.
- Minimum 2 years of experience in home health, hospice, care coordination, or case management preferred.
- Experience in sales, business development, or healthcare outreach is a plus.
- Ability to work independently while collaborating with internal and external stakeholders.
- Proficiency in using CRM tools and electronic health records to track and manage referrals.
- Ability to travel locally to assigned facilities as needed.
- Ability to pass state mandated background check.
- Physical capability to perform essential job duties and functions.
- Ability to read, write, and speak English to ensure effective communication amongst team, staff, and families.
Qualifications
This position requires a current clinical license in the states of Minnesota and North Dakota. Minimum 2 years of experience in home health, hospice, care coordination, or case management is preferred. Experience in sales, business development, or healthcare outreach is a plus. The ideal candidate should have the ability to work independently while collaborating with internal and external stakeholders, proficiency in using CRM tools and electronic health records to track and manage referrals, and the ability to travel locally to assigned facilities as needed. Additionally, the candidate must be able to pass state mandated background checks and possess the physical capability to perform essential job duties and functions. Fluency in reading, writing, and speaking English is also required to ensure effective communication with the team, staff, and families.
