Home Health and Hospice Nurse Liaison

As the Home Health and Hospice Nurse Liaison for Westminster Canterbury Richmond at Home, LLC (WCRaH), you will facilitate the coordination of care for patients transitioning to or from inpatient and outpatient hospital stays, as well as the admission of hospice or home health patients, working closely with referring physicians, social workers, families, and WCRaH.

Schedule

Full-Time, 8:30am to 5:00pm (Monday through Friday)

Pay

The hiring range for this position is $23.75 to $35.00 per hour. Actual compensation depends on experience, skills, and internal equity.

Responsibilities

• Educates potential patients, families, and referral sources on eligibility requirements and benefits of home health and hospice.
• Meets with families and patients upon request to educate them on hospice and home health and begins some of the admission paperwork.
• Facilitates the return of patients from hospitals to the appropriate level of care in collaboration with hospital discharge planners, social workers, and care coordinators.
• Advises and consults with the patient, family, and other agency personnel in a variety of settings both on and off campus, providing advocacy on behalf of the patient and their family.
• Acquires all necessary healthcare information and forwards it to the appropriate healthcare department ahead of a resident’s return to the community.
• Provides a value-added service to outside healthcare facilities, positively branding WCR as an option for individuals requiring post-acute, home health, and hospice aftercare.
• Ensures a smooth and successful transition of patients into WCRaH home health and hospice.
• Meets with potential patients at home, in the hospital, or elsewhere to educate them on the eligibility requirements and benefits of home health or hospice.
• Meets with potential patients’ families to help them through their loved ones' decision-making process.
• Obtains orders and collects pertinent medical information from physicians, nursing homes, hospitals, and/or assisted living facilities.
• Prepares the WCRaH clinical team for the new patient and sets up the formal admission.
• Attends hospice IDT meetings and home health clinical meetings.
• Responds within 24 hours to all hospice inquiries.
• Speaks to community groups, referral sources, and other audiences on topics such as hospice, home health, advance directives, and other healthcare-related issues.
• Provides nursing visits as time allows.

Requirements

• Current, unrestricted license in the Commonwealth of Virginia as a Licensed Practical Nurse or Registered Nurse.
• Current CPR certification for healthcare providers.
• Demonstrated ability to read, write, and speak English.
• Good verbal and written communication skills.
• Ability to meet deadlines and work under limited supervision.
• Valid Driver’s License in the State of Virginia with a minimum age of 21 to drive for the organization.
• Motor Vehicle Driving Record satisfactory to the Agency and its insurance providers.
• Proof of motor vehicle insurance.
• Graduate of an accredited school of professional nursing.
• Two years of nursing experience, preferably in Hospice or Home Health.
