BayCare HomeCare is seeking a skilled Home Health RN to join our team who is passionate about providing outstanding customer service to our Community. Responsibilities

• Provide hands-on care in the home for a group of patients consistently using the nursing process and clinical standards and protocols
• Oversee the activities of the multi-disciplinary team that provides care to the patient
• Monitor the provision of therapeutic services through written and verbal communication with all disciplines
• Ensure that all visits scheduled and provided are authorized and covered by physician orders
• Review patient visits weekly and provide plan to Patient Care Supervisor and team assistant, including anticipated weekend visits
• Inform the scheduler of any schedule changes and communicate caseload and patient care issues to Home Care Supervisor

Requirements

• BLS (Basic Life Support) certification
• RN (Registered Nurse) License
• Driver's License
• State of Florida driver's license required within 30 days of hire
• Associate's or Diploma Nursing (required)
• 1 year Registered Nurse experience (required)
• Bachelor's Nursing (preferred)
• 1 year Home Care experience (preferred)
• Level II screening through the Florida Care Provider Background Screening Clearinghouse, managed by the Agency for Health Care Administration (AHCA)

Schedule

• Full-Time
• 8:00 AM – 5:00 PM, Monday - Friday
• Weekend work: Every 3rd
• On Call: Yes
• Home visits in assigned service area: Pasco County
