Summary

Care for a group of patients consistently using the nursing process and clinical standards and protocols. Responsible for managing the activities of the multi-disciplinary team providing care to the patient, in addition to monitoring the provision of therapeutic services through written and verbal communication with all disciplines.

Responsibilities

• Ensure that all visits scheduled and provided are authorized and covered by physician orders.
• Review patient visits weekly and providing plan to Supervisor Patient Care and team assistant, including anticipated weekend visits.
• Inform team assistant of any schedule changes and communicates caseload and patient care issues to Supervisor Patient Care.
• Coordinate care with care team assigned to caseload, participates in care coordination communication and documentation thereof.
• Self manage visits weekly by triaging by risks associated with potential rehospitalizations.
• Participate in quarterly performance improvement action plans.
• Complete patient documentation timely as indicated by supervisor with accuracy.

Qualifications

• Required: 1 year experience as Registered Nurse or 1 year experience as Licensed Practical Nurse
• Preferred: 1 year Home Care experience
• Florida driver's license required within 30 days of hire

Education

• Required: Associate's Degree in Nursing or Diploma in Nursing
• Preferred: Bachelor's Degree in Nursing

Credentials

• Basic Life Support (BLS) certification
• Registered Nurse license
• Florida Driver's License

Schedule

• Full Time, Exempt
• Shift: 8:00 AM – 5:00 PM, Days
• Weekend Work: Every Other
• On Call: Yes
• Remote: Never
