About the Role

The Registered Nurse (RN) provides skilled nursing care in home settings, ensuring compliance with physician orders and care plans.

Responsibilities

• Deliver and coordinate patient care, monitoring and assessing patient conditions.
• Document assessments, nursing actions, and responses promptly and accurately.
• Develop, update, and implement individualized care plans.
• Recognize and respond to changes in patient needs to prevent or reduce risks.
• Educate patients and families on disease management, self-care, and prevention.
• Adhere to infection control standards and maintain professional boundaries.
• Provide patient-centered care in a compassionate manner.
• Maintain compliance with healthcare standards.

Requirements

• Active, unencumbered RN license in the practice state.
• Current CPR certification; TB test within the last 12 months.
• Proficiency in clinical assessments, documentation, and compliance.
• Ability to lift 50 pounds and perform physical tasks like walking, standing, and bending.
• Ability to travel and respond to emergency situations.
• Willingness to work in diverse environments, potentially exposed to blood, bodily fluids, and infectious diseases.

Qualifications & Skills

• Preferred: Six months of recent clinical experience.
• Preferred: Home health experience.
• Preferred: Reliable transportation.
• Excellent attention to detail, time management, communication, and problem-solving skills.
