Home Health Care Liaison

Key Responsibilities

• Develop and manage referral relationships with hospitals, rehab centers, nursing homes, clinics, physicians, social workers, and community organizations
• Drive consistent patient referrals and increase agency census
• Represent the agency professionally at community events, meetings, and outreach initiatives
• Maintain ongoing communication with referral sources to ensure satisfaction and repeat referrals
• Track outreach activity and referral results
• Work closely with intake and operations teams to ensure smooth admissions

Requirements

• Minimum 3 years of home care marketing experience
• Established referral source relationships preferred
• Strong knowledge of Medicaid, MLTCs, and private pay home care
• Excellent communication and relationship-building skills
• Self-motivated, organized, and results-driven
• Valid driver’s license and ability to travel locally
