What You Will Do
- Provide routine patient care
- Assist patients with daily hygiene activities
- Observe patients and report observations and concerns
- Complete patient records accurately in Home Care Home Base (HCHB)
- Maintain a clean, healthy environment for the patient
- Attend and participate in Interdisciplinary Group meetings, in-services, and staff meetings
- Participate in a minimum of twelve (12) hours of in-service education per year
What You Need
- High school diploma or GED
- Licensed Home Health Aide (HHA) or Certified Nurse Aide (CNA) in the State of Indiana
- Interest and ability in providing patient care in homes and facilities
- Ability to travel via your own vehicle
- Valid Indiana Driver’s License and car insurance
Salary
$20/hr - $21.50/hr plus mileage
