What an HHA at Our Hospice Will:
- Provide routine patient care in the patient’s home or long term care facility per the Home Health Aide assignment in an exemplary manner, providing comfort, care, and compassion to patients and families.
- Absorb patients with daily hygiene activities (including, but not limited to: helping patients to shower, change clothing, shave, and toilet).
- Observe patients and report observations and concerns to the primary care nurse and/or Patient Care Coordinator, Nurse Facilitator or Manager of Inpatient and Bartholomew.
- Complete patient records accurately in Home Care Home Base (HCHB) and on a timely basis.
- Aid in maintaining a clean, healthful environment for the patient.
- Attend and participate in Interdisciplinary Group meetings, in-services, and staff meetings as requested.
- Participate in a minimum of twelve (12) hours in-service education per year.
What an HHA at Our Hospice Needs:
- A high school diploma or general education degree (GED).
- A licensed Home Health Aide (HHA) or Certified Nurse Aide (CNA) in the State of Indiana.
- An interest and ability in providing patient care in homes and facilities.
- The ability to travel via your own vehicle.
- A valid Indiana Driver’s License and car insurance.
Pay:
$20/hr - $21.50/hr plus mileage
