The role involves providing direct personal care and assistance with daily activities such as bathing, dressing, grooming, and toileting, following an individualized care plan under supervision. It includes encouraging patient independence, supporting light housekeeping for a safe environment, and preparing meals aligned with dietary needs. Care delivery emphasizes infection prevention and supporting rehabilitative exercises in collaboration with therapists. Observing and documenting patients' physical, emotional, and behavioral conditions, and communicating significant changes promptly are essential duties. Team participation, offering feedback, and ensuring accurate documentation are key responsibilities. Providing compassionate, respectful, and culturally sensitive care while maintaining patient privacy is mandatory. Qualifications High School Diploma or GED with successful completion of a Home Health Aide program within 12 months, orCurrent Home Health Aide Certification (no diploma/GED required if certified). Work Environment & Skills Primarily in patient homes, with some office work in climate-controlled settings. Requires physical ability to work in unpredictable environments, stand and walk for extended periods, lift/push/pull at least 50 lbs, and assist with patient mobility. Includes travel between locations. Serves patients of all ages. Has clinical access to unrestricted patient information (Level 3).
