About the role
The Home Health Aide (HHA) is responsible for providing various care services necessary for attending to the personal needs, support, and health status of participants. The primary purpose of this position is to assist with transporting PACE/LIFE participants to and from the PACE/LIFE Center and to other appointments, ensuring their safety and comfort while waiting for transport or appointments. The HHA will communicate with drivers and the center, providing relevant information regarding the status of appointments, participants, and any problems or issues.
Responsibilities
- Assist with transporting PACE/LIFE participants to and from the PACE/LIFE Center and other appointments.
- Promote the safety and comfort of participants while waiting for transport or appointments.
- Communicate with drivers and the center, providing updates on participant status, appointments, and any issues.
- Provide one-to-one care with patients in their homes or in a clinical setting.
- Assist participants with activities of daily living, including toileting, showers, eating, and grooming.
- Maintain participant safety by identifying hazards and emergency situations and initiating corrective action.
- Provide behavioral supervision or support, such as reality orientation, reminders for nursing and medication instructions, diet restrictions, and emotional reassurance.
- Assist with general activities, including conversing with participants, recreational activities, meal and snack serving, and housekeeping.
- Assist in maintaining physical activity, exercise, and walking routines for participants.
- Provide nursing care as assigned in a safe manner, consistent with organizational policies and procedures.
Minimum Qualifications
- High school diploma or equivalent.
- Transportation and human services experience preferred.
- CPR trained or willingness to be trained.
- Minimum of one (1) year of documented experience working with a frail or elderly population. If less than one year of experience, must receive appropriate training from the PACE organization upon hiring.
- Valid driver’s license in good standing preferred.

Pay
Wage range: $16.94 – $22.87 per hour
