Home Care Services Supervisor

Essential Duties And Responsibilities

• Executes tasks focused on quality care, relationship management, and operational efficiency.
• Serves as primary liaison between MAH and its clients, as well as between the Masonicare continuum and both current and potential clients as it relates to home care services.
• Creates, reviews and updates individualized care plans in collaboration with clients, families, and healthcare professionals.
• Serves as expert resource to caregivers and support staff. Timely addresses issues and concerns and escalates to Executive Director appropriately.
• Carrries out intake activities and tracking of referrals, and conducts necessary follow-up.
• Conducts home care-related consultations and assessments to ensure safety and client satisfaction.
• Maintains accurate and timely documentation related to client care, visits, and communications.
• Attends community and continuum events as invited/requested to embed Masonicare at Home in the communities we serve.
• May assist with recruitment, interviewing, and selection of care staff.
• Communicates effectively and tactfully with staff and clients, recognizing their age, cultural diversity, needs, abilities and physical condition.

Minimum Qualifications

• Education: Bachelor’s degree in a health-related field strongly preferred. Will consider relevant experience in this field in lieu of degree.
• Experience: Private duty homecare experience preferred with a strong knowledge of local markets and the geriatric field.
