Home Care Registered Nurse (RN) Hartford Region

About the role

Trinity Health Of New England At Home provides in‑home nursing, social work, and therapy services as a Catholic, mission‑driven nonprofit affiliated with Johnson Memorial Hospital and the Trinity Health system. Serving the Hartford Region, Home Care RNs deliver primary nursing care to home‑based patients per physician orders, develop care plans, supervise aides and LPNs, and strive for top‑decile client outcomes.

Responsibilities

• Plan and prioritize visits based on client needs and physician orders.
• Assess clients at admission and key intervals and document accurately.
• Develop and update care plans with measurable goals in collaboration with physician and client.
• Provide therapy using approved techniques, exercises, and modalities.
• Coordinate care and referrals with other disciplines as needed.
• Educate clients and caregivers on therapy programs and home exercises.
• Monitor progress and report changes promptly to the physician.
• Ensure compliance with laws, regulations, and agency policies.
• Complete documentation at point of care in a timely manner.

Requirements

• Graduate of an approved nursing education program.
• Licensure as a Registered Nurse in the State of Connecticut.
• One (1) year experience as a professional acute care nurse.
• Home health care experience preferred but not required.
• Ability to work with minimal supervision and maintain professional relationships with clients and families.
• Valid driver’s license and reliable transportation required.

Pay

$10,000 sign‑on bonus, paid out over two years.

Schedule

Monday – Friday, 8 am – 5 pm; one weekend every 4 weeks and two rotating holidays per year. No on‑call requirements.
