Home Care Registered Nurse (RN) Hartford

Full-time Home Health Registered Nurse (RN) position based in the Hartford region.

About the role

As a Home Health Registered Nurse, you’ll deliver skilled nursing care in the comfort of patients’ homes, creating individualized care plans, administering medications, educating patients and families, and supporting recovery and independence. This role offers the chance to practice at the top of your license with greater flexibility, autonomy, and meaningful one-on-one patient relationships.

Schedule

Monday – Friday, 8 am to 5 pm, with a weekend rotation every 4 weeks and 2 holidays per year.

Pay

Competitive RN pay plus a $10,000 sign-on bonus.

Responsibilities

• Deliver skilled nursing care in patients’ homes
• Develop and implement individualized care plans
• Administer medications and treatments
• Educate patients and families on care and recovery
• Support patient independence and recovery

Requirements

• Graduate of an approved nursing education program (BSN preferred)
• Licensure as a Registered Nurse in the State of Connecticut
• One (1) year of experience as a professional acute care nurse
• Home care experience preferred but not required
• Current Driver’s license and reliable transportation
