Home Care Registered Nurse (RN) Hartford Region
Full-time Home Health Registered Nurse (RN) position based in the Hartford region.
Schedule
- Monday – Friday, 8 am to 5 pm
- Weekend rotation every 4 weeks
- 2 holidays per year

About the Role
As a Home Health Registered Nurse, you’ll deliver skilled nursing care in the comfort of patients’ homes. Responsibilities include creating individualized care plans, administering medications, educating patients and families, and supporting recovery and independence. This role offers flexibility, autonomy, and meaningful one-on-one patient relationships, whether you're experienced in home health or transitioning from hospital, acute care, or skilled nursing.

Requirements
- Graduate of an approved nursing education program (BSN preferred)
- Licensure as a Registered Nurse in the State of Connecticut
- One (1) year of experience as a professional acute care nurse
- Home care experience preferred but not required
- Current Driver’s license and reliable transportation
