Home Care Registered Nurse (RN)

Vna & Hospice of the Southwest Region is seeking a full-time Home Care Registered Nurse (RN) in Rutland County. Responsibilities

• Assess the needs of patients to create and implement their plan of care.
• Serve as a case manager for assigned patients, adhering to evidence-based practices, state and federal regulations, and company policies.
• Maintain up-to-date and accurate documentation of nursing care provided.
• Ensure adherence to infection control standards and stay updated on high-tech skills such as IV use and maintenance, wound VAC, and complex wound care.
• Carry out visits with patients in a timely manner and educate patients and caregivers on health matters.
• Coordinate with various physicians, providers, team members, and vendors to fulfill the recommended plan of care.
• Aid in patient discharges or transfers and complete necessary paperwork and reports.

Qualifications

• Registered Nurse (RN) license.
• Two or more years of nursing experience preferred.
• Self-direction and initiative, with the ability to work collaboratively with colleagues.
• Experience with electronic medical records.
• Flexible schedule, including rotational weekends, holidays, and on-call availability for both hospice and home care patients.
• Exceptional bedside manner and communication skills.
• Compassionate and friendly demeanor.
• Valid Vermont Driver's License and car insurance.
