About the role
Vna & Hospice of the Southwest Region is seeking a full-time Home Care Registered Nurse (RN) in Rutland County. Responsibilities
- Assess the needs of patients to create and implement their plan of care.
- Serve as a case manager for assigned patients, adhering to evidence-based practices, state and federal regulations, and company policies.
- Maintain up-to-date and accurate documentation of nursing care provided.
- Ensure adherence to infection control standards and stay updated on high-tech skills such as IV use and maintenance, wound VAC, and complex wound care.
- Carry out visits with patients in a timely manner and educate patients and caregivers on health matters.
- Coordinate with various physicians, providers, team members, and vendors to fulfill the recommended plan of care.
- Aid in patient discharges or transfers and complete necessary paperwork and reports.
Qualifications
- Registered Nurse (RN) license.
- Two or more years of nursing experience preferred.
- Self-direction and initiative, with the ability to work collaboratively with colleagues.
- Experience with electronic medical records.
- Flexible schedule, including rotational weekends, holidays, and on-call availability for both hospice and home care patients.
- Exceptional bedside manner and communication skills.
- Compassionate and friendly demeanor.
- Valid Vermont Driver's License and car insurance.
