Provide personalized patient care primarily in home settings.
Responsibilities
- Collaborate with care teams, patients, and families to develop and implement care plans.
- Assess clients and deliver clinical and non-clinical care.
- Provide training and education to promote safety and independence.
- Document care activities and ensure quality standards are met.
- Develop and follow care plans.
- Train patients and families for safety and independence.
- Work collaboratively with care teams to ensure optimal patient outcomes.
Requirements
- Active RN license in Maine.
- 1-2 years of relevant experience.
- Active CPR certification.
- Valid driver’s license, reliable transportation, and clean driving record.
- Must meet COVID, booster, and flu vaccination requirements unless medically exempted.
