Home Care Registered Nurse (RN)
About the role
VNA & Hospice of the Southwest Region is looking to hire a full-time Home Care Registered Nurse (RN) in Bennington County. This home health nursing care position earns a competitive wage and offers up to a $10,000 sign on bonus.
Responsibilities
- Assess patient needs and create and implement their plan of care.
- Serve as a case manager for assigned patients in accordance with evidence-based practices, state and federal regulations, and company policies.
- Actively manage the patient's treatment and needs.
- Maintain up-to-date and accurate documentation of nursing care provided to ensure the effective integration of information.
- Ensure infection control standards are maintained and stay educated on high-tech skills, including IV use and maintenance, wound VAC, and complex wound care.
- Carry out visits with patients in a timely manner and educate patients and caregivers on health matters.
- Coordinate with physicians, providers, team members, and vendors to fulfill the recommended plan of care.
- Assist with patient discharges or transfers and complete all required paperwork and reports.
- When on call, be responsible for home care and hospice patients.
Qualifications
- Registered Nurse (RN) license.
- 2+ years of nursing experience preferred.
- Self-direction and initiative; ability to work collaboratively with colleagues.
- Experience with electronic medical records required.
- Willingness to work a flexible schedule, including rotational weekends, holidays, and on call for both hospice and home care patients.
- Exceptional bedside manner and communication skills.
- Compassionate and friendly demeanor.
- Valid Vermont Driver's License and car insurance.
