Home Care Registered Nurse

Responsibilities

• Participate in interdisciplinary care planning to develop personalized patient care plans.
• Provide professional nursing services aligned with established care plans to promote patient recovery and independence.
• Accurately document patient care activities to ensure compliance, facilitate coordination, justify reimbursement, and support quality improvement.
• Demonstrate positive interpersonal skills and personal accountability in completing work assignments and professional growth.

Qualifications & Skills

• Successful completion of nursing education from an approved school.
• Current RN licensure in the applicable state.
• At least one year of clinical nursing experience in acute or long-term care; home health experience preferred.
• Strong clinical nursing skills and knowledge of medical-surgical principles.
• Effective communication skills, both oral and written.
• Proficiency in computer use and familiarity with various software tools.
• High-level problem-solving, critical thinking, and reasoning abilities.
• CPR certification.
